DATA THAT WE COLLECT

  • Identity Data like your email address, first name, last name, marital status, title, date of birth, and gender.

  • Contact Data like your home address, email address and telephone numbers.

  • Health Data is your medical history collected with medical & emotional questions. Clients and prospective clients tell us about their general health, fertility health, mental health, medical conditions and medication.

  • We do not collect Sensitive Data such as your race or ethnicity, religious or philosophical beliefs, sexual orientation, criminal convictions and offences.

  • Financial Data is not collected. All payments are collected via Sum Up, BACS, PayPal or PayPal.me which should be GDPR compliant.

  • Transaction Data may include details about payments made by you for treatments or any other products/services.

 

By providing us with your personal information you warrant to us that you are over 18 years of age.

 

WHY DO WE COLLECT PERSONAL INFORMATION

We use the personal information provided by you

  • To contact you, to provide your requested services or information.

  • To make and rearrange appointments.

  • To create a treatment plan tailored to your needs.

  • For marketing purposes like sending you a newsletter, only if we have your explicit consent.

  • We may send you promotional information about third parties which might be of interest to you.

  • We need some information to satisfy legal, accounting and medical requirements.

 

STORING AND PROTECTING YOUR DATA

  • Your personal data will be stored and accessed by staff at Fertility Wellness only.

  • Your data will not be shared to any third-party companies.

  • We retain your personal data electronically, in our database and on a password protected computer.

  • Your consultation forms with medical history information are kept either electronically on a password protected computer or as paper files in a locked cabinet.

  • We might access your personal information on our mobile phone which is passcode protected and regularly cleaned up.

 

Please note that no electronic transmission over the internet or information storage technology can be guaranteed to be 100% secure, so we cannot guarantee that hackers, cybercriminals, or other unauthorized third parties will not be able to defeat security and improperly access, steal or modify your information.

Our company is hosted on the Wix.com platform. Wix.com provides us with the online platform that allows us to display and sell our services to you. DATA RETENTION

  • For tax purposes we are obliged to keep basic information about customers (including Contact, Identity, Financial and Transaction Data) for six years after your last transaction with us.

  • Under Public Liability we are obliged to keep consultation files on record for seven years after your last treatment.

  • After seven years, your files will be erased, shredded or burnt.

 

DISCLOSING INFORMATION
We will not sell, distribute, lease or share your personal information to third parties unless authorized by you or are required by law to do so.
